Some mouths have reached a point where treating one tooth at a time no longer makes sense: years of grinding have shortened every tooth, old fillings and crowns are failing in a rolling sequence, several teeth are already gone and the bite has collapsed. Full mouth reconstruction is the discipline of stepping back, deciding what the finished mouth should look like and function like, and then sequencing crowns, implants, root treatments, gum care and sometimes orthodontics to get there in a sensible order. A good fit when
- Generalised wear from grinding or acid erosion that has shortened the teeth and changed the face.
- Multiple failing crowns, fillings or root-treated teeth that keep breaking in turn.
- Several missing teeth alongside damaged ones, so replacement and repair need planning together.
- A bite that has collapsed or shifted, causing jaw discomfort, chipping or an aged appearance.
- Patients who want the whole picture priced and sequenced rather than a series of surprises.
- Good enough general health to undertake a long course of treatment, with gum disease stabilised first.
Think twice, or choose another route, when
- Two or three isolated problems; those are simply treated individually.
- Uncontrolled gum disease or active decay, which must be brought under control before any rebuilding.
- Untreated grinding without a commitment to a night guard; new ceramics will be ground down like the old teeth were.
- Anyone unable to commit the time; reconstructions take many appointments over months.
- Expectations of a finished mouth in a few weeks, which is not how bone, gums and complex laboratory work behave.
The process
How it works at Wimpole Street
- 01
Diagnostic phase
Full examination, photographs, radiographs, a CT scan where implants are likely, gum charting and a bite analysis. Study models or a digital scan are mounted so the technician can see how the jaws relate. This phase carries its own fee and produces the plan.
- 02
Design and preview
The technician builds the proposed teeth in wax or digitally at the new bite height. Where the front teeth are involved, a trial smile is placed over your own teeth so the length and shape can be judged in the mirror before anything irreversible happens.

Stabilisation
Gum treatment, extractions of hopeless teeth, root canal treatment and any grafting come first. Implants are placed and left to heal. Teeth to be crowned are prepared and fitted with long-term provisionals at the new bite.

Living with the provisionals
You wear the provisional teeth for weeks or months while implants integrate and the jaw muscles adapt to the new bite. Anything that does not feel right is changed at this stage, not after the final ceramics are made.

Definitive restorations
Crowns, bridges, implant crowns and any dentures are made from the proven provisional design and fitted in sequence, usually one arch or segment at a time, with the bite refined at each step.

Protection and maintenance
A night guard is made, a hygiene schedule set and reviews booked. A reconstruction is finished on the day the maintenance plan starts, not the day the last crown goes in.

Outcomes
What the result looks like
A completed reconstruction restores the height of the bite, so the lower face regains its proportions and the lips their support; teeth that had worn to stumps are the length they were in your twenties; and chewing spreads evenly across both sides rather than concentrating on whichever teeth still meet. Patients often notice the disappearance of jaw ache and headaches they had stopped attributing to their teeth.
Longevity is a portfolio rather than a single number. Individual crowns and implants carry their own expected lifespans, ten to fifteen years and more, and within a large reconstruction some component will need attention sooner than others. The plan we give you includes a maintenance schedule and an honest note of which elements are most likely to need work first.

Looking after the result
- Wear the night guard every night; it is the single most important protection for a reconstructed bite.
- Attend hygiene appointments at the interval set in your plan, usually every three to four months for the first year.
- Clean around implants and under bridges daily with the interdental brushes and floss threaders we show you.
- Report any chip, high spot, looseness or new sensitivity promptly; small adjustments prevent larger repairs.
- Keep the annual review with radiographs so implant bone levels and crown margins are monitored.
Informed consent
Risks and honest limitations
Every treatment has trade-offs. These are the ones we discuss at the consultation so that a decision is made with full information rather than after the fact.
- Cost and duration. Plans stretch over many months and the total is substantial. We stage the fee to match the stages of treatment and set out everything in writing first.
- Component failure. In a mouth with twenty or more restored units, the chance that something chips, loosens or fails over a decade is high. Repairs are usually straightforward but they are a real future cost.
- Nerve problems in crowned teeth. A proportion of teeth prepared for crowns later need root canal treatment.
- Implant failure. A small percentage of implants do not integrate or are lost later; smoking, diabetes and gum history raise the odds. Our warranty covers replacement subject to terms and regular review.
- Adaptation to a new bite. Most people adjust within weeks; a few need the bite refined more than once, which is why the provisional stage matters.
- Occasionally a tooth planned for a crown proves unsaveable once opened and the plan has to change. We tell you when that happens and revise the fee accordingly.

It depends entirely on what needs doing. Several crowns and a root treatment might run to a few thousand pounds; multiple implants and a full set of crowns run considerably higher; full-arch implant work higher still. The diagnostic phase produces an itemised figure before you commit to anything.
Six to eighteen months is typical. Implant healing and the provisional phase account for most of that; the appointments themselves are spread out rather than continuous.
No. Provisional crowns, bridges or dentures are in place throughout, and they are made at the proposed new bite so you are effectively test-driving the result.
