Dental implants in Marylebone: a titanium root, a crown, and the patience to let bone do its work
An implant is a small titanium screw placed into the jaw where a root used to be. Over the following months the bone knits onto its surface, a process called osseointegration, and once that has happened a connector and a crown are fitted on top. Done well, the result functions like the tooth that was lost and nothing is done to the teeth either side. Done in a hurry, or in the wrong mouth, implants fail. At Wimpole Street the £2,950 fee covers the implant and its crown, planning uses a 3D scan, and we will say plainly if bone volume, smoking or medical history make another option wiser.

Is it right for you?
A good fit when
- A single missing tooth where the neighbours are healthy and you would rather not have them drilled for a bridge.
- Several gaps in one arch that would otherwise need a large partial denture.
- A loose lower denture you are tired of; two or four implants can lock it in place.
- Good general health, healthy gums and enough bone, confirmed on a CT scan.
- Non-smokers, or smokers prepared to stop for the healing period and ideally beyond.
Think twice, or choose another route, when
- Heavy smokers who will not pause; nicotine roughly doubles the failure rate in most published series.
- Uncontrolled diabetes, recent radiotherapy to the jaw, or certain bone medications, which slow or prevent integration.
- Active gum disease anywhere in the mouth, which must be treated and stable first.
- Insufficient bone where grafting is declined or impractical; a bridge or denture may then be the realistic route.
- Anyone expecting a fixed tooth within days; the biology takes months and shortcuts increase risk.
The process
How it works at Wimpole Street
- 01
Consultation and 3D scan
Examination, photographs and a cone-beam CT scan showing bone height, width and density and the position of nerves and sinuses. A detailed medical history is taken. You receive a written plan with the fee, the timeline and any grafting itemised separately.
- 02
Preparation, if needed
A failing tooth may need extracting and the socket left to heal for two to three months, or a graft placed to preserve the ridge. Gum treatment, if required, is completed now.

Placement
Under local anaesthetic, the gum is opened, a channel is drilled to the scan-based plan and the implant is seated. A single site takes about an hour. The days afterwards feel similar to a straightforward extraction.

Healing
Osseointegration usually takes three months in the lower jaw and up to six in the upper, where bone is softer. A temporary tooth can be worn if the site shows. We review at two weeks and again before restoring.

Restoration
Once the implant is stable, a scan is taken and a technician makes the crown. It is screwed or cemented onto an abutment, and the bite and contacts are adjusted.

Maintenance
A review at one year with a radiograph to check bone levels, then annually alongside hygiene visits. Implants do not decay, but the gum around them can inflame, and catching that early is what keeps them.

Outcomes
What the result looks like
A successful implant crown is indistinguishable in function from a natural tooth: you chew on that side without thinking and floss around it as you would anything else. Because the root is replaced, the bone that would otherwise shrink after an extraction is largely preserved, which matters for the lip and cheek over the years.
Appearance depends heavily on the gum. Where tissue and bone are intact, the crown emerges like a tooth. Where bone has already been lost, the crown may be a little longer than its neighbour, and we show you this on the plan rather than at the fitting.
Survival in the literature sits above ninety per cent at ten years for well-selected patients. That is not a promise for your tooth; smoking, grinding, poor cleaning and some medical conditions pull the odds down, and a small proportion of implants fail to integrate in the first months.
Afterwards
Looking after the result
- For the first week, eat soft food, avoid the site and rinse gently with warm salt water after meals. Do not smoke.
- Clean the implant crown with a soft brush twice a day and use floss or an interdental brush around it daily.
- Keep the reviews at two weeks, before restoration and at one year, even if everything feels fine.
- Wear a night guard if you clench or grind; an implant has no ligament to cushion it.
- Report any looseness, bleeding or swelling promptly; early peri-implant inflammation is treatable.
Informed consent
Risks and honest limitations
Every treatment has trade-offs. These are the ones we discuss at the consultation so that a decision is made with full information rather than after the fact.
- Failure to integrate. Typically two to five per cent of implants do not bond to bone in the first months and have to be removed. A second attempt after healing usually succeeds, and our warranty covers replacement in that scenario, subject to terms.
- Peri-implantitis. Gum and bone infection around an established implant, driven by plaque and made more likely by smoking and a history of periodontitis. It is the main reason implants are lost after the first year.
- Nerve or sinus involvement. The lower jaw nerve and the upper sinus run close to implant sites. The CT scan is how we keep clear of both, but altered sensation, usually temporary, remains a recognised risk.
- Time and additional procedures. Grafting, socket healing and slower integration can stretch a plan from four months to a year.
- Mechanical problems. Screws can loosen and crowns can chip, particularly in heavy grinders. Repairable, but not free once outside warranty.
- Warranty terms. We stand behind our implant work for a defined period, subject to terms, attendance at reviews and hygiene visits, and not smoking. It is a repair-or-replace warranty, not a promise that nothing will go wrong.

Questions patients ask
Three to six months from placement to final crown is typical. Add two to three months if a tooth has to be removed first, and more if grafting is needed.
Placement is done under local anaesthetic. Afterwards, expect bruising-type discomfort for a few days, comparable to an extraction. Sedation can be arranged for anxious patients.

We ask you to stop for at least two weeks before and eight weeks after placement, and ideally for good. Smokers lose implants at roughly twice the rate of non-smokers.
